Legacy Studio Medical Aesthetics

Welcome to Legacy Studio Medical Aesthetics. These Terms and Conditions govern your use of
our website and services. By accessing or using our website or services, you agree to be bound
by these Terms and Conditions and our Privacy Policy.

Services
Legacy Studio Medical Aesthetics provides aesthetic and health-related services, including but
not limited to medical consultations, treatments, and procedures. All services are provided
subject to availability and the terms outlined herein.

Appointment Scheduling
Appointments for services can be scheduled through our website, by phone, email, or in person.
By scheduling an appointment, you agree to attend at the designated time and understand that
failure to attend may result in cancellation fees as outlined in our policies.

Payment
Payment for services is due at the time of appointment, unless otherwise agreed upon in
advance. We accept payments via credit card, debit card, or other forms of payment specified
by us. You authorize us to charge your provided payment method for all charges incurred by
you.

Medical Information
You agree to provide accurate and complete medical information relevant to your treatment. Our
healthcare providers will rely on the information you provide to determine appropriate
treatments. It is your responsibility to inform us of any changes to your medical history or health
status.

Cancellation and Rescheduling
Appointments must be cancelled or rescheduled at least 24 hours in advance. Cancellations
made less than 24 hours before the scheduled appointment time will incur a cancellation fee of
$50.00, depending on the cause of cancellation. We require credit card information to hold
appointments. By providing your credit card details, you authorize us to charge your card in
accordance with our cancellation policy.

Confidentiality
We are committed to protecting your privacy and confidentiality. All personal and medical
information you provide to us will be handled in accordance with our Privacy Policy. However,
please be aware that no transmission of data over the internet is entirely secure.

Limitation of Liability
To the fullest extent permitted by law, Legacy Studio Medical Aesthetics and its affiliates shall
not be liable for any indirect, incidental, special, consequential, or punitive damages, including
but not limited to damages for lost profits, revenue, goodwill, or data arising out of or related to
your use of our services.

Governing Law and Jurisdiction
These Terms and Conditions shall be governed by and construed in accordance with the laws
of the province of Ontario and the federal laws of Canada applicable therein, without regard to
its conflict of law provisions. Any dispute arising under these Terms and Conditions shall be
resolved exclusively by the courts located in Ontario.

Changes to Terms and Conditions
We reserve the right to update or modify these Terms and Conditions at any time without prior
notice. Changes will be effective immediately upon posting on our website. Your continued use
of our services after changes are posted constitutes your acceptance of the updated Terms and
Conditions.
