Microneedling With Diluted Botox for Skin Rejuvenation
Nicole's Take: This is one of the treatments I'm most excited about — and I want to
explain exactly why, while also being completely transparent about where the
research stands.
Most people think of Botox as a wrinkle-relaxer that freezes muscles. But when you
dilute it and deliver it into the skin itself (not the muscle) using microneedling, it does
something completely different: it improves the actual quality of your skin — texture,
pore size, oiliness, fine lines, tightness, and even redness. Think of it this way:
traditional Botox treats specific wrinkles; microneedling with diluted Botox
treats the skin itself.
The research we've reviewed — 15 studies spanning over 1,900 documented cases
— is overwhelmingly positive. Every single study showed improvement, and not a
single one reported serious side effects. Here's what stands out to me:
For skin texture, pores, and oil control: The largest quantitative study (62
patients) showed significant, measurable improvement in skin texture, roughness,
and pore size at 90 days. Oil production was reduced at every time point measured
— 1 week, 1 month, 2 months, and 3 months — with 91% of patients reporting
50–75% improvement in oiliness. Pore size decreased by 7%, skin lines decreased
by 10%, and pigmentation decreased by 9%. Peak results occur at about 4 weeks,
with benefits lasting at least 3 months.
For the lower face and neck: One study directly compared microbotox to the well-
known "Nefertiti lift" technique — and microbotox was actually more effective for
improving jowls and neck fullness, with 100% patient satisfaction. This is the study
that convinced me this technique offers something traditional Botox simply cannot.
For facial redness and rosacea: Two gold-standard double-blind, placebo-
controlled trials (the highest quality research design) confirmed that intradermal
Botox significantly reduces facial redness compared to placebo — with a 44%
reduction in redness severity in one study. One trial even showed it reduces the
density of visible blood vessels under the skin.
Where I want to be honest with you: While the results are consistently positive,
most of the studies are small (12–62 patients), and many don't have a control group.
We have only 2 double-blind placebo-controlled trials, and those focused specifically
on rosacea, not general skin rejuvenation. The microneedling delivery method
specifically has been tested in only 4 clinical studies — the rest used intradermal
injection, which delivers the Botox to the same skin layer but through a different
technique. The science tells us the mechanism is the same, but more studies using
microneedling specifically would strengthen the evidence. There is also no
standardized protocol yet — dosing, dilution, and treatment intervals vary between
practitioners.
My bottom line: The science behind this treatment is genuinely compelling. The
biological mechanisms are well understood, the results are consistently positive
across every study, the safety profile is excellent, and the real-world clinical
experience (1,867+ documented cases from a single expert alone) confirms what the
research shows. This is not a treatment based on hype — it's based on a growing
body of evidence that Botox does far more than relax muscles. I offer this treatment
with confidence, and I expect the evidence to only get stronger as more clinical trials
are completed.
